In April 2007 a new research unit will start: „Prioritizing in Medicine: A Theoretical and Empirical Analysis in consideration of the public health insurance system“. The Deutsche Forschungsgemeinschaft (DFG - German Research Foundation) will finance the unit with 1,58 million euros over the next three years. The DFG research unit 17 scientists from 12 universities and research institutes work within this team of researchers. They focus on the building of rankings when it comes to questions of medical provision. Does the growing offer of medical treatments lead to the abandonment of therapies due to the fact that the financing of all these possible measures is not given? Does this mean, that children should be preferred to older people? Is prevention more important than medical attendance? Should society finance luxurious therapies for a few patients?
Doctors and politicians decide these and similar questions on a daily basis. Mostly, they act according to practical and ad hoc reasoning, according to a “hidden prioritization”. Defined concepts and procedures for a systematic information management are missing. Without these it is difficult to provide transparent and comprehensive decisions.
The researchers will look for prioritizations that respect the interests and wishes of all parties involved, patients, doctors and citizens. On the one hand, decisions for medical treatments should be acceptable on theoretical and ethical grounds – especially for affected people. On the other hand they have to be conform to set rules and must be institutionally realizable.
Outstanding scientists and academics from different areas are brought together in the research unit. In eleven sub-projects they explore the problem from different perspectives. One string of research follows ethical, philosophical, legal and economical aspects while the other approach focuses on psychological, empirical and medical aspects.
Sub-project of Jacobs University In addition to Dr. Adele Diederich, Professor of Psychology, two other Jacobs University professors support the team: Dr. Petra Lietz, Professor of Quantitative Research Methods, and Dr. Margrit Schreier, Professor of Empirical Methods in the Humanities and Social Sciences. Their sub-project alone accounts for 589.000 euros of DFG funding. The professors seek to determine the decision rules responsible for the preference of certain medical treatments and patients or patient groups. Their empirical study will aim at discovering hidden prioritization for different parties involved. Different from any current approaches they focus systematically on needs and interest of patients, physicians, and the general public. Several methods are utilized, from qualitative interviews to a representative survey to conjoint analysis and models such as random utility models.
