Transcript Request Form

You need to log in with your campusnet credentials in order to be able to fill out the form

Please note: If you order the transcript between Thursday and Sunday, it will be available by the following Tuesday. If you order the transcript between Monday and Wednesday, it will be available by Friday. The transcript will be sent to your mailbox. If you live off Campus, you will have to collect it in RLH 115b.
