LEAVING FORM FOR GRADUATING STUDENT
Before you submit this form, please make sure you have no pending or outstanding balances or items with the following departments: Student Financial Services, Housing, IRC/Library, Accounting (for Masters and PhD students). In addition please make sure you have returned your transponder to the main gate and can upload the receipt in this form. After this form is submitted, the different departments will confirm your check-out to Registrar Services. Only after these confirmations have been received and processed, Registrar Services will start to issue the final documents. You will be personally contacted once they are ready.
Please note this process can take up to 6-8 weeks and can vary depending on the confirmations from other departments.
