LEAVING FORM FOR GRADUATING STUDENT

Before you submit this form, please make sure you have no pending or outstanding balances or items with the following departments: Student Financial Services, Housing, IRC/Library, Accounting (for Masters and PhD students).  In addition please make sure you have returned your transponder to the main gate and can upload the receipt in this form.  After this form is submitted, the different departments will confirm your check-out to Registrar Services. Only after these confirmations have been received and processed, Registrar Services will start to issue the final documents.  You will be personally contacted once they are ready. 

Please note this process can take up to 6-8 weeks and can vary depending on the confirmations from other departments.

Please provide the information regarding the last graduation requirement you've fulfilled. The date you provide here will be the date on your diploma (for students graduating after June 11).

Graduation Documents

Your final documents include a diploma (if it was not already received/picked up), final transcript, diploma supplement, disenrollment confirmation, institutional letter, brochures, etc.
If you haven't used your free 15 copies during your studies you can request extra transcript copies to be added to your final documents. If you have already received 15 copies, you can have copies for 5€ each. (Applicable to only bachelor students).
If you would like to be disenrolled before the end of the semester (i.e. Aug. 31st or Jan. 31st.), please specify the date above. If you do not specify an earlier date, you will automatically be disenrolled by Aug. 31st or Jan. 31st, respectively.
